Mandatory
insurance would be provided from two sources: through the workplace, such as the
"play or pay" proposals or employer-based insurance as used in Hawaii; and
directly through the government for those on pensions or unemployment. Voluntary insurance
plans with supplemental benefits would be available to anyone at an additional
out-of-pocket cost, similar to the indemnity and service-benefit plans of Blue Cross-Blue
Shield. Indemnity plans, which also act as claims processors, reimburse directly to the
consumer while service-benefit plans reimburse the provider. The burden of administrative
control and the profitability of fiscal control was transferred from the Russian
government to either the consumer of competing indemnity plans or the provider (hospital,
third-party, physician) in service-benefit plans. |
