ADRs were identified as the 4th to 6th leading cause of death among hospitalized patients in the USA in 1994, ahead of pneumonia and diabetes. Studies on ADR and hospital admissions estimate that ADR account for 5% of Hospital admissions, occur in 11% of hospital patients, and double the costs, length of stay and mortality risk. Nearly 50% are potentially preventable and possible less than 1% are reported.
