1) Anderson R, May R. Infectious diseases of humans. Oxford: Oxford University Press; 1991 (757 pages).
A very good classic textbook which focuses both on concepts and mathematics of models. It gives a detailed account of all aspects of herd immunity summarising much of the seminal work of the authors. It includes a special section on the herd immunity aspects of metazoan infections.
2) Fine P. Herd immunity: history, theory, practice. Epidemiol Rev 1993; 15:265-302.
A comprehensive literature review with very interesting sections on the historical evolution of the concepts related to herd immunity, on the present theoretical developments and on practical issues for a number of specific diseases, particularly in relation to infection eradication.
3) Giesecke J. Modern infectious disease epidemiology. London: Arnold, 1994 (256 pages).
A good introductory book of epidemiology with reference to infectious diseases all the way through. It includes a very comprehensible chapter on the basic concepts of herd immunity focusing mainly on the dynamics of epidemics (p. 109-23).
