Preventive and
screening programs must be evaluated in terms of their coverage of high-risk versus
low-risk individuals. In order to evaluate and monitor the effectiveness of their service coverage, population-based preventive programs need the following: - Accurate, updatable population registry using a (encrypted) personal health identification number for each enrollee - Person-based information for each preventive service delivered The data MCHP receives from such programs do not contain any personal identifiers (e.g. names, addresses, PHIN) Also see: Delivering Prevention Generating such a profile may raise questions such as: 1. Why are mammography screening rates for women so low? How can these be raised? 2. Do regions with varying rates have different service delivery systems or are there other influencing factors? 3.
