For each
indicator we identified specific eligibility criteria (for patients), and
the recommended care, based on clinical practice guidelines and previous
research. To be considered eligible, patients had to have at least one
physician visit in the study period (2001/02). The complete list of indicators and their definitions are presented in Table 1.
